Health Partners of Western Ohio will continually strive to improve the health of our community by providing a patient-centered “health care home” that addresses the mind, body and spirit of our patients.

Our History

Health Partners of Western Ohio is a private, non-profit corporation that was formed by a group of community residents and health professionals in Lima, Ohio. The impetus for the formation of Health Partners came from the activities of the Healthy People 2000 in partnership with the City of Lima. Healthy People 2000 identified access to primary care services as a top priority and out of this program grew the formation of the Southside Health Center Development Committee and MUA/HPSA designations were obtained for five census tracks in south Lima. Health Partners was originally formed as Allen County Health Partners in October 2002 and applied to become a “New Access Point” (NAP) in December 2003, receiving funding December 1, 2004.
